What to do if hurt on campus

Athletes
1. If you get hurt on campus during practice or in general please get in and see your AT.
    a. To see who your AT is:
          i. Valleywillroll.com>Inside Athletics>Athletic Trainers
2. Once you talk/schedule to see your AT you will be evaluated.
    a. Once the evaluation is completed, your AT will fill out the accident claim form.
    b. The AT will also discuss the plan of action:
        i. Rehab
        ii. Doctor Visit (if necessary)
            1. Imaging (if necessary)
            2. Surgery (if necessary)

3. Depending on the plan of action the AT will need to get you the secondary insurance-accident insurance card (printed or electronically) as well as the claim instruction sheet so you can take it to the necessary provider(s).
      a. You will still need to take your primary insurance as well as it will need to be billed first and the EoB from them will need sent to the school’s secondary-accident insurance.
